Students with disabilities need a supportive team during their transition to adulthood. This consortium enables collaboration to help them succeed. Teachers, administrators, agency personnel, and parents are welcome to attend. See below for dates and topics.

About the event

All educators and professionals who serve and help students with disabilities in Transition are invited to join the Heart of Georgia Transition Consortium. Goals for the training include improving transition services and post-school outcomes for students with disabilities by improving transition plan compliance, establishing and implementing best practices in transition services, and empowering students with disabilities through self-determination initiatives.

  • September 11, 2025 - Resource Mapping

  • November 11, 2025 - Self-Advocacy & Student-Led IEPs

  • January 8, 2026 - Family Involvement

  • April 16, 2026 - Writing the Summary of Performance
