Best Practices in Transition Plan Writing

Best Practices in Transition Plan Training will give educators responsible for writing transition plans the tools and skills necessary to write effective and compliant transition plans. Educators will learn about the required components of the transition plans and where to locate those components on the transition plan. Instruction will also be given on writing appropriate post-secondary goals and including activities and services to reach those goals. Procedural safeguards and documentation of evidences regarding transition plans will be discussed, as well as how to monitor transition plans for the required components of compliant transition plans.

Day 2—Taking it to the next level. This half-day follow-up is a practical session that will guide participants in using peer reviews to monitor the compliance and fidelity of the transition plans. To make the most of this session, each participant must bring 2 personally identifiable information-redacted transition plans.
